Given the gist of each case perfectly and reviewed it well for the 1st question.

Many of them haven't done 2nd question I appreciate that she has made elog regarding the case of Acute renal failure and has presented well.

3rd and 4th questions regarding cardiovascular case is reviewed well in an order of chief complaints symptoms and the plan of treatment 

5th question has been done exceptionally well regarding the technical knowledge being gained during the pandemic and showing towards general medicine department is quite impressive.

This is a case of Acute renal failure presented well in the chronological order and is updated time to time with the treatment .Investigations of ECG, ABG,haemogram are being posted pictorally for a quick review.Taken care of not reviewing patients details.

49 yr old female with generalized weakness and vomtings based on present and past illness has been diagnosed to have chronic intestinal nephritis secondary to multiple myeloma.

Bonemarrow aspiration technique has been presented well.Treatment is updated time to time.

Case of DKA WITH AKI

Information of vitals regarding the medication to stabilize BP has been noted.

Day to day investigations are noted and the progression of day to day treatment is quite impressive

Diagnosis is done well and investigations suggesting to urinary tract infections are impressive only draw back of disclosing patients details need to be corrected.

 Case of Acute kidney injury secondary to urosepsis

Diagnosed well and hyperkalemia has been treated based on investigations of serum electrolytes. Systemic examination has been done accurately.

Question 4

      ACUTE KIDNEY DISEASES 

Sudden in onset primarily reversible.Generally patient came to OPD with c/o oliguria lower abdominal pain
decreased urine output with burning micturition, fever vomitings pitting type of oedema bilaterally in lower limb. Investigations 🔎show low albumin levels
Normal hemoglobin concentration not associated with anemia.
At times AKI are associated with other diseases like pancreatitis hepatomegaly

SOLUTION: Renal transplantation is not required Dialysis can be performed time to time 
            
CHRONIC KIDNEY DISEASES 
       
     It is sudden in onset, predominantly irreversible. General c/o patients are muscle aches, fever, generalized weakness, vomitings non bilious, normal or increased urinary output no pedal edema if at all present is associated with periorbital edema which indicates severe kidney damage Haemoglobin conc. decreased anaemia seen, kidney size is reduced,changes in lipid profile like decreased HDL increased LDL

      SOLUTION: RENAL TRANSPLANTATION IS NECESSARY. TIMELY DIALYSIS REQUIRED 

Question 5:

This  pandemic had a major impact on education being unable to attend offline postings eblogs has been of great help. It's been 6 weeks of GM postings .During first day of my posting I was unable to understand the complications of particular disease and its treatment, but day by day I am getting better at grasping the concepts.The case sheets being given to us makes us more enthusiastic and helps us in furthering our knowledge. 
Sharing knowledge with our peers and interacting with them regarding cases has been of a lot help.
Assigning intern to a batch of five students is a lot helpful for knowing the basics of history taking and correcting our mistakes. 
 I would like to thank GM department for creating such a platform to progress our clinical knowledge.
